We will listen, explain options, confirm eligibility with your doctor, and provide 24/7 nurse support once enrolled.
Home Health: Home health is for people who need intermittent skilled care at home and are considered homebound, meaning it takes considerable effort to leave home. A doctor’s order is required, but you can call us first, and we will help coordinate with your physician. You can also ask for VNA by name or request a VNA Nurse Care Coordinator while in the hospital or a facility.
