When Is Home Health the Right Choice?

Home health care may be appropriate if you or a loved one:

  • Is recovering after a hospital stay or surgery
  • Is managing a chronic condition such as heart disease, diabetes, or COPD
  • Needs wound care, IV therapy, or medication management
  • Has experienced a fall or mobility decline
  • Is at risk for avoidable hospital readmissions
  • Requires therapy services to restore strength, speech, or independence

What Is Home Health Care?

Home health care is physician-directed, skilled medical care provided in the home on an intermittent basis. It focuses on rehabilitation, stabilization of chronic illness, and restoration of independence. Services are delivered by licensed clinicians and therapists who evaluate progress, coordinate care, and adjust plans as needed.

Home health care is not long-term custodial care. It is structured, goal-oriented support designed to help patients improve, stabilize, or safely manage ongoing conditions at home.

Please note: VNA Home Health does not currently provide services for:

  • Pediatric patients (ages 0–21)
  • Maternity diagnoses
  • Psychiatric or primary behavioral health conditions
