Our VNA Home Health Care team provides comprehensive, skilled services in the home. Home health services focus on rehabilitation and restoration of independence after a surgery, an acute illness, as well as management and stabilization of chronic diseases. VNA provides intermittent care through the expert skills of our trained and certified staff, to assist patients, families, and caregivers in the comfort and familiarity of their own homes.

Physical Therapist

Occupational Therapist

  • Evaluates your home environment and ability to functions safely
  • Recommends solutions and techniques to increase independence

  • Identifies new ways to do activities of daily living

Speech Therapist

  • Evaluates speech limitations and disorders
  • Maximizes functional independence in communication and swallowing
  • Reduces the risk of aspiration and choking
  • Improves memory and thinking

Medical Social Worker

  • Assess needs and identifies limitations, barriers and/or safety concerns
  • Provides education and counseling to patients and families
  • Advocates for patients’ needs
  • Connects patients and families to necessary resources and support in the community

Home Health Aide

  • Provides activities of daily living support
  • Assists with personal care, such as bathing, dental care, using the toilet, dressing and undressing
  • Helps with moving to and from furniture
  • Measures and records patient’s temperature, pulse and blood pressure when ordered

How to Receive Skilled Home Health Care

Contact VNA & Hospice at 831-375-9882 to arrange for services. Ask your physician for VNA or ask to speak with a VNA Nurse Care Coordinator while in the hospital, skilled nursing facility or assisted living facility. Our team members are available 24 hours a day, 7 days a week.

Cardiac Care

The Cardiac Care program is a home health program where care is delivered by a specialized clinical team for people living with cardiac diagnoses. The program focuses on relief of symptoms and stress related to cardiac diagnoses. The goal is to improve the quality of life for the patient and family by educating, demonstrating, addressing physical and emotional needs.


  • Comprehensive in home nursing services.
  • Prompt comprehensive assessments to obtain the appropriate care needs, following specific protocols creafted in collaboration with local cardiologist.
  • A customized plan of care utilizing healthy living education.
  • Utilization of the VNA cardiac team members including your cardiologist and primary care physician to help you and your caregiver manage medications, diet and other aspects of treatments.
  • Increase your knowledge regarding the cardiac diagnosis, including new diagnosis education.
  • Improve your quality of life and reduce unwanted hospitalizations.

Connections Program

The VNA Connection is a professional or self-referral program which offers assistance with health care needs assessment and decision making. It also supports the individuals with coordination of community services during any stage of health including acute, chronic or life-threatening conditions.

These services are at no cost to individuals and families.

Orthopedic Program

Specific protocols crafted in collaboration with local orthopedic physician and hospital joint programs; bringing a clinical care directly to meet your joint replacement needs including a pre home safety evaluation.

Palliative Care Program

Palliative Care is specialized medical care for people living with a serious illness. It focuses on providing relief from the symptoms and stress of the illness, whatever the diagnosis. The goal is to improve quality of life for both the patient and family by addressing physical, emotional, social, and spiritual needs.

Are There Eligibility Requirements?

  • Anyone with a serious illness who is over the age of 21 that meets the home health homebound criteria*
