Our specially trained home health therapists provide evidence based, hands-on rehabilitation with an emphasis on preventing re-hospitalizations and ensuring you have the best outcome possible.

Our program is flexible, providing care as needed, and is adjusted according to your individualized needs and in accordance with your physician’s treatment plan.

Are There Eligibility Requirements?

  • Anyone with an orthopedic diagnosis who is over the age of 21 that meets the home health homebound criteria
