- Home health: You may qualify if you are under a doctor’s care, need part-time skilled nursing or therapy, and it is a real effort to leave home. Ask your physician to refer you to VNA or call our referral line to request an evaluation. Medicare is eligible when the criteria are met.
- Hospice: Hospice is right when a physician certifies a life expectancy of six months or less if the illness follows its usual course. You or a family member can call us to review goals and see if hospice is the best fit. Medicare benefits are available for eligible patients.
