The VNA Connections Program is a professional or self-referral program that offers assistance with health care needs assessment, community resources, long-term care planning, and decision-making. We provide services to patients with any stage of health, including acute, chronic, or life-threatening conditions.
These services are at no cost to individuals and families.
VNA Connections Program is NOT a nursing program.
The services provided by the Connections Program are not medical in nature.
