Referral Information

VNA utilizes the expertise of our Referral Department’s Patient Care Planning Nurses to allow for easy access to our services. Referrals can be made by:

• Hospitals
• Clinics
• Physicians
• Other Healthcare providers
• Community agencies

All referrals require the physician that you select to follow your care at home to be notified, and for VNA Home Health services to be ordered by your physician.

For Medicare, and most private insurance patients to be admitted to home care certain requirements need to be met. Those requirements include for the patient to be primarily homebound, have a medical need for services, and require skilled and intermittent care of registered nurses and qualified therapists. We can help determine if you qualify for our services.

New Medicare Requirements from the Health Care Affordability Act require patients to be seen face to face by a Home Care Certifying Physician within ninety (90) days prior to starting home care services, or thirty (30) days after starting services. VNA will help you to ensure that this requirement is met.

The Patient Care Planning Nurses will call you when the referral is received from the Physician to verify your personal information and process the referral. Our goal is to see our patients within 24 hours of receiving the doctor’s orders for care.

In addition to our Patient Care Planning Nurses, we also provide nurses, called Transitional Care Managers to work with you at your bedside to streamline your move from the Hospital or Nursing home to your home. These nurses come to the facilities to assist the discharge planners in preparing for your home care needs. Ask to speak with the VNA Transitional Care Managers if you are hospitalized or in the nursing home.
