HOSPICE

VNA is looking for a caring and compassionate Nurse Practitioner to join our caring and compassionate Hospice/Palliative Care team. Responsibilities for this position include clinical practice, education, consultation, and research. This position works interdependently with the multidisciplinary team, guiding members of the multidisciplinary team, (e.g., RNs, LVNs, HHAs, PTs, OTs, STs, MSWs, nursing students and newly employed RNs), as appropriate, and supervises patient plans of care including education on Hospice Benefit as appropriate.  The NP may perform home visits when physicians/NP level visit is needed for patients in absence of their primary physician of record.

Salary Range: $76.53 – $83.22 Hour

Education/Experience/Qualifications: Possession of a license to practice as a Nurse Practitioner in the State of California.  Current CPR certification. Master’s degree or equivalent certification required.  CA requires a Furnishing License and DEA Certificate or obtain within one year of placement in position.  Knowledge of Medicare regulations. Excellent knowledge of fundamentals of pain and symptom management.  Excellent interpersonal and communication skills.  Fluent in Spanish including medical terminology preferred. The Hospice Clinical Supervisor assumes the authority, responsibility, and accountability for directing the home care and/or hospice programs under the supervision of the Clinical Manager and as back up to the Clinical Manager in their absence. Actively participates in the management of clinical aspects and quality assurance of the home care and hospice programs, and the selection, supervision, and optimal utilization of qualified personnel. Implements policies and procedures. Facilitates clinical management of patients.  Coordinates and attends patient care planning conferences. Participates in patient care assessment, planning, intervention, and evaluation. Participates in licensing surveys and reviews. Serves on or leads various committees. Promotes favorable public relations.

Annual Salary Range: $120,500.00 - $132,000.00

HOME HEALTH

Works with physicians in identifying and coordinating patient/family/caregiver home care needs and providing skilled supportive care to patients and their families.  Utilizes the nursing process to provide skilled care and comfort and maximizes the quality of life to the patient, family, and caregiver.  Refers and coordinates other disciplines and specialties, transfers to other levels of care and services with other providers.

Education/Experience/Qualifications:  Possession of a license to practice as a Registered Nurse in the State of California.  One year of experience as a Registered Nurse.  Experience in a medical‑surgical facility within the last three years and one to two years of recent home care experience preferred.  Possession of current CPR certification.

The Physical Therapist Assistant (PTA) will provide skilled therapy services to our patients in order to increase their safety and independence in their homes by executing physical therapy interventions to meet the established physical therapy goals as established by the evaluating and/or supervising Physical Therapist (PT).

Salary Range: $40.00 - $43.50 Hour

Education/Experience/Qualifications:  Graduate of a physical therapy assistant program approved by the Physical Therapy Examining Committee, or equivalent education and experience recognized by the Physical Therapy Examining Committee.  Minimum of one year of experience as a Physical Therapist Assistant.  Possession of current CPR certification.

Under a plan of treatment approved by the MD, the Speech-Language Pathologist (SLP) assists patients in attaining maximum potential through application of speech therapy principles. The SLP works to prevent, assess, diagnose, and treat speech, language, social communication, cognitive-communication, and swallowing disorders.  The licensed SLP will work with the home health Multi-disciplinary team, the patient, and their caregivers to reach the optimal patient’s treatment goals and will re-evaluate per Medicare requirements to justify needed services.  Additionally, the SLP will attend and participate in participant care conferences and staff meetings as required.

Salary Range: $63.74 - $69.31 Hour

Education/Experience/Qualifications: Graduate of a speech therapy curriculum accredited by the Council on Medical Education of the American Medical Association. Possess a Speech Therapist license from the California Board of Medical Quality Assurance. Minimum of one year of experience as a speech therapist. Possess a current CPR certification.

Through directing and managing a multi-disciplinary team of home health care clinicians and support staff, Clinical Managers focus on care coordination, financial management, and resource utilization to yield cost-effective outcomes that are patient-centric, safe, and provided in the least restrictive setting. Clinical Managers provide the environment where patients, families, and stakeholders are viewed as essential members of the team, costs are contained, and enhanced outcomes are achieved.  Clinical Managers provide a fluid and dynamic practice that is most effective when it changes and adapts with the challenges of the healthcare system. Stewardship of the healthcare dollars, safe transitions of care, evaluating patient adherence, and consistent stakeholder communication are critical interventions that Clinical Managers employ, while maintaining a primary and consistent focus on quality of care and patient self-determination. Have an understanding of utilization management and payer requirements in home health.

Salary Range: $125,000 - $140,000 Annual

Utilizes the nursing process to provide comfort, care and maximize the quality of life for the patient and family.

Education/Experience/Qualifications: BSN preferred. Two years of experience as a professional nurse in a medical-surgical facility within the last three years preferably with one year in palliative nursing or two years of recent home care experience with one year in palliative home care. The Director of Patient Care Services (DPCS) provides oversight and support of all patient care and associated personnel for assigned program(s). This oversight includes patient and personnel assignments, coordination of care, coordination of referrals, assessment of patient needs, and ensuring the plan of care is developed, implemented and updated. The DPCS is responsible for the services provided by the home health and hospice programs and strives to provide the highest quality of care through all service lines. The DPCS is responsible for staff engagement, development and performance, including policy, regulatory and compliance knowledge and adherence. The DPCS is always available during operating hours (or designates a similarly qualified alternate) and participates in all activities relevant to assigned programs including setting/developing and executing strategic goals, budgets and staffing models.

The Director of Patient Care Services is the primary Clinical Manager for the Home Health/Hospice Programs and is essential for managing the complex, interdisciplinary care of home health/hospice patients. The DPCS has joint responsibility with the COO/Administrator for seeing that patient care standards are met by participating in development and implementation of agency objectives to obtain and meet the agency vision and mission statements.

Salary Range: $142,000 - $154,410.36 Annual

Education/Experience/Qualifications: Possession of a current RN license to practice in the state of California and current CPR certification. Three to five years’ experience as a Home Health Supervisor or Hospice Manager. Ability to learn to effectively operate computer hardware and software programs.

Hourly Salary Range: $60.82 - $65.98

ADMINISTRATION

The Director of Development (DoD) is responsible for implementing the strategic vision of CCVNA through developing and executing fundraising plans and securing individual, corporate, and foundation support, and public and private grants. The DoD is responsible for all functions of the development department, including formulation and implementation of fundraising goals, optimizing complete and accurate donor records, and assuring timely acknowledgment of gifts and donor recognition.

The DoD will manage the growth and sustainability of new donors, grants, and gifts. This role will collaborate with internal departments to meet regulatory tracking and reporting requirements for funds received. The DoD will collaborate with the CEO and the Board of Directors to build an effective development committee and ensure volunteers are being supported and deployed appropriately.

Key Responsibilities

  • Develop and implement comprehensive one-year and five-year fundraising plans to meet the organization’s revenue goals.
  • Identify and cultivate major gift, corporate, and foundation prospects.
  • Design and deploy a robust planned giving program.
  • Plan and execute special events that engage donors and raise funds for the organization.
  • In partnership with the development team: research, cultivate, manage and grow public and privately funded grant opportunities.
  • Manage and lead a team of fundraising professionals, providing mentorship, training, and support as needed.
  • Collaborate with the Communications team to develop and execute effective donor communications and marketing materials.
  • Work closely with the Board of Directors and Executive Director to develop fundraising strategies and goals.
  • Facilitate exceptional board/staff partnerships and ensure the Board of Directors has the support and assistance needed for the execution of their responsibilities.
  • Promote an internal culture of philanthropy in partnership with the Board of Directors.
  • Communicate fundraising priorities to employees/clinicians and train clinicians on introducing donation opportunities to clients.
  • Monitor and analyze fundraising data to evaluate performance and adjust strategies as needed.

 

Annual Salary Range: $115,000.00 - $120,000.00

Education/Experience/Qualifications:

  • Minimum 7-10 years of experience as a successful fundraising professional
  • Bachelor’s degree in nonprofit management, business administration, related field or proven relevant experience in a related field

  • Excellent written and verbal communication skills including the ability to write compelling donor communications and marketing materials

Under the direction of the Business Development Manager, the Account Liaison Representative (ALR) will use their goal driven aptitude and drive to meet or exceed target sales goals, by leveraging VNA’s long standing legacy and mission of providing exceptional patient care through a variety of service lines. The ALR will present and educate all services VNA offers to the community to varied audiences including participation in community events enhancing brand awareness. The ALR will work collaboratively with their various VNA colleagues, acting as the Liaison between agency and clients.

Annual Salary Range: $64,480.00 - $70,115.35

Status: Full-Time (Exempt)
