Is responsible for the development and implementation of the Spiritual Care Program for hospice home care. Provides patient and family spiritual care and coordinates care with the IDT and patient's pastor as indicated. Actively participates in community education regarding the spiritual care of the terminally ill. Facilitates and makes recommendations in relationship to questions regarding ethical protocols. Facilitates development of a volunteer chaplaincy.

Education/Experience/Skills/Qualifications: Masters of Divinity or equivalent in pastoral related issues and skills. Recognition and standing in accepted spiritual community. Demonstrated experience in pastoral and spiritual counseling. Demonstrates knowledge of concepts and care related to death and dying.

Works with physicians in identifying and coordinating patient/family/caregiver home care needs and providing skilled supportive care to patients and their families.  Utilizes the nursing process to provide skilled care and comfort and maximizes the quality of life to the patient, family and caregiver.  Refers and coordinates other disciplines and specialties, transfers to other levels of care and services with other providers.  Case manages patient care within the guidelines of the various payor sources.  Prepares patient/family for discharge when appropriate.

Pay: $51.29 - $55.64 per hour

Education/Experience/Skills/Qualifications: Possession of a license to practice as a Registered Nurse in the State of California.  Two years of experience as a professional nurse with home care/hospice experience preferred.  Possession of current CPR certification.

Assesses the social and emotional factors related to patient’s illness, need for care, capacity, and potential to cope with problems of daily living.  Works with team members in development of treatment plans and participates in conferences.

Pay: $35.66 - $38.68 per hour

Education/Experience/Skills/Qualifications: Master's degree from a school of social work accredited by the Council Social Work Education.  One year of social work experience in a health care setting, preferably with a home health agency.   Possession of a current CPR certification.

HOME HEALTH

This position conducts admissions for all organization programs including: conducts a thorough assessment of the client to determine client needs, completes OASIS documentation as required, completes all agency admit paperwork and submits by next business day, establishes plan of care in collaboration with physician and patient, obtains MD orders for care, communicates patient needs and status to MD and to interdisciplinary team, makes appropriate referrals to other disciplines and delivers skilled nursing care to the patient.

Education/Experience/Skills/Qualifications: Possession of a license to practice as a Registered Nurse in the State of California. One year of experience as a professional nurse in a medical-surgical facility within the last three years and one to two years of recent home care experience. Possession of current CPR certification.

Under a plan of treatment approved by a physician, assists the patient in attaining maximum potential through application of physical therapy principles.  Provides consultation to other health team members.  Consults with patient's family and caregivers.  Evaluates home environment and makes recommendations.

Currently licensed by the Physical Therapy Examining Committee of the California Board of Medical Quality Assurance.  Minimum of one year of experience as a physical therapist, preferably in an acute rehabilitation setting.

ADMINISTRATION

The CFO is responsible for all financial policy and programs in the agency and is a key member of the management team. Provides strategic leadership to support the agencies strategic, financial, clinical, and operating goals. Prepare and present budgets, financial analysis, provide accurate and timely financial reports, manage finance, revenue and payroll teams. Establish and monitor internal controls, protect assets of the agency, manage cash flow, administer payroll, revenue cycle management (including billing and collecting), and contract management oversight. Coordinate audits and oversee necessary annual filings for state and federal programs. Ensure compliance with established accounting principles and all state and federal fiscal intermediary and accrediting regulations.

Education/Experience/Skills/Qualifications: BA/BS in Accounting / Finance or related Business Degree, CPA strongly preferred. Any equivalent combination of training, education, and experience that provides the required knowledge, skills, and abilities. Knowledge of accounting principles, practices, standards, laws and regulations. Minimum 7 years of experience in Healthcare setting. Minimum of 5 years of management experience (preferably with home health or hospice background). Knowledge of governmental payer sources requirements and regulations.

Assumes the authority, responsibility and accountability for directing the home care and/or hospice programs under the supervision of the Clinical Manager/DPCS/COO. Actively participates in the management of clinical aspects and quality assurance of the home care and hospice programs, and the selection, supervision and optimal utilization of qualified personnel. Implements policies and procedures. Facilitates clinical management of patients. Coordinates and attends patient care planning conferences. Participates in patient care assessment, planning, intervention and evaluation.

Education/Experience/Skills/Qualifications: Possession of a license to practice healthcare in the State of California and current CPR certification required, with at least 2 years of recent experience in home care or hospice, preferably with 1 year of supervisory experience or equivalent training in a home health or hospice agency. Nurse Practitioner preferred. Excellent communication, leadership, organizational, decision-making, and time-management skills. Working knowledge of: Title XXII, HIM-11, Medi-Cal requirements, agency policies and procedures.

Strong working knowledge of Home Health and Hospice operations with some Public Health knowledge preferred. Excellent interpersonal and communication skills. Demonstrated organizational skills. Must have the ability to establish and maintain cooperative relationships with professionals and community groups.
