Is responsible for the development and implementation of the Spiritual Care Program for hospice.   Provides patient and family spiritual care and coordinates care with the IDT and patient's spiritual leader as indicated.  Actively participates in community education regarding the spiritual care of the terminally ill.  Facilitates and makes recommendations in relationship to questions regarding ethical protocols.  Develops and maintains bereavement records.  Responsible for the Bereavement Program. Continues bereavement with the families for at least 13 months.  Oversee the Community grief support groups.

Education/Experience/Skills/Qualifications:    Masters of Divinity or equivalent in pastoral related issues and skills.  Recognition and standing in accepted spiritual community.  Demonstrated experience in pastoral and spiritual counseling.  Demonstrates knowledge of concepts and care related to death and dying. Has knowledge of coordination of support groups. Experience in health care delivery system and general program management. Ability to write procedures. Competency in oral and written communications. Non-judgmental and flexible. Currently licensed by the Physical Therapy Examining Committee of the California Board of Medical Quality Assurance.  Minimum of one year of experience as a physical therapist, preferably in an acute rehabilitation setting.

This position conducts admissions for all organization programs including: conducts a thorough assessment of the client to determine client needs, completes OASIS documentation as required, completes all agency admit paperwork and submits by next business day, establishes plan of care in collaboration with physician and patient, obtains MD orders for care, communicates patient needs and status to MD and to interdisciplinary team, makes appropriate referrals to other disciplines and delivers skilled nursing care to the patient.

Under the direction of the Patient Care Planner (PCP) Supervisor, this position is responsible for collecting patient data and performing new patient referral procedures. The PCP LVN participates in identifying appropriate candidates for home care services.  This position is responsible for accepting and processing all referrals from patients, families, hospitals, physicians and other referral services when requested.  The PCP LVN executes tasks within the PCP department as directed and within the LVN Scope of Practice. This includes the completion of required documentation and processes to ensure that start of care is initiated in a timely manner according to regulatory guidelines. Demonstrates and provides excellent customer service to referral sources, patients, and family members. The PCP LVN is aware of internal and external services, facilities, and community resources, and develops and maintains rapport with members of the medical community. The PCP LVN monitors the progress of the referral through the referral process, and reports to the appropriate supervisor and care coordination team when changes in information, and data received may impact the intake and referral process.

Under the leadership of the Director of Community Services, the Community Health Nurse will focus on disease prevention and health education.  Provide immunizations to prevent or control the spread of communicable disease.  Delivers health and wellness services to employee populations and community groups.  Encourage participants to take responsibility for their health through education, disease management programs and effective use of medical services.  Assignment in public, corporate and school settings.  Will work with clients of all ages, and members of the community, including vulnerable or underserved populations.  Supports all Community Services programs.

The operational clinical support position requires a diverse skill set to assist with daily clinical operations and administrative work including but not limited to; data acquisition and analysis as directed by the clinical management or quality assurance department(s), archiving and organizing materials related to clinical training, staff development and state/federal compliance.  Additionally, skill sets in Coding, Excel and PowerPoint will be utilized to assist management staff with content preparation and clinical practice.  This position will support all agency departments through knowledge of the agency’s total operations while assisting predominately within the clinical, staff development and compliance departments.

Performs general office duties to include but not limited to: filing, mailing, photocopying, and faxing. Receives and processes phone calls timely and accurately, utilizing proper customer services skills. Fulfills ad hoc report requests. Stocks and tracks medical and office supplies. Prepares admit packets (charts) and clinical bags/boxes. Processes incoming and outgoing MD orders via fax and electronic communication and uploads documents into patients charts in a timely manner. Assists with printing, tracking, and auditing medical records requests and documents. Provides support for other departments and supervisors as assigned, performing all functions required within those roles.
