Is responsible for identifying, coordinating and implementing patient/family nursing care needs and providing supportive care to patients, families and caregivers. Utilizes the nursing process to provide comfort, and care and maximizes the quality of life to the patient and family.

Education/Experience/Skills/Qualifications: Registered Nurse: Two years of experience as a professional nurse in a medical-surgical facility within the last three years or one year of recent hospice or home care experience preferred. HOME HEALTH

Is responsible for identifying and coordinating patient/family nursing care needs and providing supportive care to patients, families and caregivers. Conducts staff training as needed. Directs and coordinates care of palliative patients including the appropriate placement of the patient under VNA & Hospice service lines based on patients changing needs. Utilizes the nursing process to provide comfort, care and maximize the quality of life for the patient and family.

Education/Experience/Skills/Qualifications: BSN preferred. Two years of experience as a professional nurse in a medical-surgical facility within the last three years preferably with one year in palliative nursing or two years of recent home care experience with one year in palliative home care. Possession of a license to practice as a Registered Nurse in the State of California. Possession of current CPR certification.

Two years of recent home care experience with one year in cardiac home care. Possession of a license to practice as a Registered Nurse in the State of California. Possession of current CPR certification.

Works with physicians in identifying and coordinating patient/family/caregiver home care needs and providing skilled supportive care to patients and their families. Utilizes the nursing process to provide skilled care and comfort and maximizes the quality of life to the patient, family and caregiver. Refers and coordinates other disciplines and specialties, transfers to other levels of care and services with other providers.

Education/Experience/Skills/Qualifications: Possession of a license to practice as a Registered Nurse in the State of California. Two years of experience as a professional nurse in a medical-surgical facility within the last three years and one to two years of recent home care experience. Possession of current CPR certification. BSN, PHN preferred.

ADMINISTRATION

Performs general office duties to include but not limited to: filing, mailing, photocopying and faxing. Receives and processes phone calls timely and accurately, utilizing proper customer services skills. Fulfills ad hoc, report requests. Stocks and tracks medical and office supplies. Prepares admit packets (charts) and clinical bags/boxes. Processes incoming and outgoing MD orders via fax and electronic communication and uploads documents into patients' charts in a timely manner. Assists with printing, tracking and auditing medical records requests and documents. Provides support for other departments and supervisors as assigned, performing all functions required within those roles.

Education/Experience/Skills/Qualifications: High school diploma or GED. Computer knowledge and skills are required, including experience in data entry and proficiency in EMR. Microsoft Office Word, Excel and Outlook programs. A working knowledge of medical terminology and experience/education in handling medical records is preferred. At least one to two years of home health office experience is preferred. Proficiency in all functions of information processing including: input/output, quality assurance and record keeping. Excellent verbal, written and customer service skills.

Under the direction of the Director of Compliance, the Quality Assurance Performance Improvement (QAPI) Nurse will review OASIS/HIS documentation for accuracy related to quality of care, visit utilization, documentation of communication between disciplines/Physicians and adherence to the established Plan of Care. Responsible for working with Clinical Managers/Supervisors and staff to educate and update any change in regulation or process, identifying trends that require Performance Improvement Projects, to ensure the highest standards of practice in providing patient care and improving Quality Outcomes (OBQI).

Education/Experience/Skills/Qualifications: A current license to practice as a Registered Nurse or Licensed Vocational Nurse in the State of California. If performing patient visits, possession of current CPR certification is required. Minimum of 2 years of experience in Health Care, preferably Home Health/Hospice. OASIS Certification within one year of hire is preferred. Excellent verbal and written communication skills.
