HOME HEALTH

The Clinical Educator is responsible for developing, implementing, and evaluating comprehensive orientation, training, and professional development programs for clinical, clerical, and management staff within the Home Health department. This role ensures staff competency, regulatory compliance, and supports quality improvement initiatives across the agency. The educator collaborates closely with leadership to identify training needs, support new hires, and promote best practices in care delivery.

The RN Case Manager works collaboratively with physicians and the interdisciplinary care team to assess, plan, coordinate, and deliver skilled home health care to patients and their families. This role focuses on providing supportive, patient-centered care that promotes comfort, safety, and quality of life. The nurse utilizes the nursing process to manage patient care, coordinates services with internal and external providers, and ensures compliance with payer guidelines. The RN Case Manager also prepares patients and caregivers for discharge when appropriate and connects them with community resources to support continuity of care.

Hourly Salary Range: $60.92 - $66.08

Status: Full-Time

Schedule: Monday – Friday w/ rotating weekends

  • This position is based in Monterey, CA and requires daily travel throughout the Monterey Peninsula and surrounding coastal communities.  Candidates must reside within a reasonable commuting distance that allows efficient coverage of the assigned territory

Under a plan of treatment approved by a physician, the Occupational Therapist (OT) assists the patient in attaining maximum potential through application of occupational therapy principles. Provides consultation to other health team members. Consults with patient's family and caregivers. Evaluates home environment and makes recommendations.

Hourly Salary Range: $65.16 - $70.68

Status: Full-Time & Per Diem

Under a physician-approved plan of care, the Physical Therapist assists patients in achieving their maximum functional potential through the application of physical therapy principles and practices. The role includes providing consultation to interdisciplinary healthcare team members, as well as educating and collaborating with patients’ families and caregivers. The Physical Therapist also evaluates the patient’s home environment and makes appropriate recommendations to support safety and independence.

Hourly Salary Range: $65.16 - $70.68

Status: Per Diem

The Clinical Supervisor directs and manages a multidisciplinary team of home health clinicians and support staff to ensure delivery of patient-centered, safe, and cost effective care in the least restrictive setting.  Clinical Supervisors provide the environment where patients, families, and stakeholders are viewed as essential members of the team, costs are contained, and enhanced outcomes are achieved.  Clinical Supervisors provide a fluid and dynamic practice that is most effective when it changes and adapts with the challenges of the healthcare system. Stewardship of the healthcare dollars, safe transitions of care, evaluating patient adherence, and consistent stakeholder communication are critical interventions that Clinical Supervisors employ, while maintaining a primary and consistent focus on quality of care and patient self-determination.

Responsibilities: 

  • Provides direct supervision, performance evaluations and disciplinary action of all assigned staff. Obtains input from Program Managers and Department leaders where appropriate. Evaluates and ensures that competency and compliance requirements are met for assigned staff.
  • Monitors staff assignments, caseloads, and team productivity. Promotes efficient utilization of resources to achieve desired clinical outcomes and within episode reimbursement.
  • Develops and maintains a system of patient tracking and financial forecasting. Ensure OASIS review and coding results in appropriate reimbursement for the patient’s clinical condition. Refers and discusses high utilization and problem prone cases with Director of Clinical Services and Quality Assurance Staff.
  • Provides the Director of Clinical Services with census, staffing, team productivity and other reports as requested. Identifies staffing patterns and needs of assigned team.
  • Works collaboratively and in support of colleagues throughout the agency.
  • Ensures staff orientation and development programs are enacted.

Annual Salary Range: $123,512 - $135,617

The Clinical Coordinator is responsible for identifying and coordinating nursing care needs for patients and their families, while providing ongoing supportive care to patients, caregivers, and loved ones. In addition, the Clinical Coordinator directs and coordinates care for palliative patients, ensuring appropriate placement within VNA service lines as patient needs evolve. Using the nursing process, the Clinical Coordinator focuses on delivering comfort, comprehensive care, and maximizing quality of life for both patients and families.

Hourly Salary Range: $63.13 - $68.49
