Case Manager and Admission RNs

Our professional staff of nurses work with physicians in identifying and coordinating patient/family/caregiver needs and providing skilled supportive care to patients and their families. Utilizes the nursing process to provide skilled care and comfort and maximizes the quality of life to the patient, family and caregiver. Refers and coordinates other disciplines and specialties, transfers to other levels of care and services with other providers.

Intake Supervisor

Oversees the management of the Intake department. Represents the Intake department in meetings with other departments. Is responsible for the intake and referral of patients into the organization. Accepts referrals from patients, families, hospitals, physicians and other referral sources. Manages the insurance verification process and reviews the face to face process at time of referral for compliance. Provides information about services and facilities. Develops and maintains rapport with members of the medical community to identify ways of best caring for patients. Must have current and in good standing RN license.

Home Health Assistant Clinical Supervisor

Under the guidance of the Clinical Supervisor, this position’s responsibilities will include triaging phone calls from patients and families, physicians, field staff and other customers. Will provide support to field staff by providing case management and patient care consultation and/or training. Will be a resource to the Leadership Team and will assist with projects, staff development, administrative assignments and clinical support as needed. Must have current and in good standing RN license.

Wellness Program Supervisor

Under the guidance of the Director of Community Services, the Wellness Program Supervisor oversees the wellness portfolio of services and contracts. Our Wellness [at] Work program travels to client’s worksites to implement biometric screenings, complete Health Risk Appraisals (HRAs), provide health education, immunizations and wellness coaching. This position promotes, develops and grows worksite wellness programs within our community through establishing and meeting client wellness goals. Remains current with industry trends and mandates affecting wellness programs.

Payroll – A/P Specialist

The Payroll-A/P Specialist performs duties and tasks associated with maintenance of payroll records and preparation of payroll checks. This position audits and verifies information recorded on employee timecards. This position will maintain and update payroll records that track the accumulation and use of sick, vacation, holidays or PTO for employees. The Specialist will prepare vouchers for disbursement of retirement plans, health plans, union groups, garnishments and other payroll related items. The Specialist will prepare and review any required quarterly and annual payroll reports. The Specialist performs the duties and functions required to process all bills, invoices and expenses incurred.
