Outreach & Education at CHISPA communities for Seniors

VNA continues to provide resources to the local south county CHISPA communities with the utilization of the Latino Mobile Unit. The purpose of the site visits is to provide tools and resources for community members to have a medical plan during COVID-19. All the information will be in Spanish. This plan can include the following:
- Advance Directives formulary
- Having a list of prescribed medications on hand
- Planning a trip to the hospital, how to be prepared?
- Planning medical decisions
- Planning for your pets
